Pharmaceutical companies are deploying artificial intelligence across drug discovery, clinical trials, manufacturing quality control, and pharmacovigilance at an accelerating pace. Most are navigating AI governance with two incomplete maps: the ISO 42001:2023 certification framework on one hand, and an evolving set of FDA guidance documents on the other. Treating these as separate compliance workstreams is the single biggest governance mistake pharma organizations are making right now.

I have worked with regulated organizations across life sciences, medical devices, and biopharma for years, watching companies build parallel systems that satisfy neither their certification auditors nor their FDA investigators. This article lays out what a genuinely integrated pharma AI governance program looks like.

Why Pharma AI Governance Is Different

Three things make pharma different from every other sector navigating AI governance.

First, patient safety is not a secondary consideration. An AI system that misclassifies a manufacturing deviation or misinterprets a safety signal in pharmacovigilance can cost a patient their life. That asymmetry shapes every governance decision.

Second, regulatory expectations in pharma are not aspirational. The FDA does not issue frameworks as suggestions. ISO 42001 is a voluntary standard. FDA requirements are not. Any pharma AI governance program that conflates these two things is building on an unstable foundation.

Third, pharma operates in a GxP environment where AI does not get special treatment. 21 CFR Part 11 electronic records requirements, ALCOA+ data integrity principles, and Computer Software Assurance are the baseline.

The FDA Regulatory Landscape in 2026

The FDA has issued several guidance documents that directly affect how pharma companies must govern AI. Understanding which guidance applies to which AI use case is non-trivial.

January 2025 Final Guidance: Artificial Intelligence in Drug and Biological Product Development

This is the most significant AI-specific FDA document to date. It applies to AI used in drug development submissions, including AI used to analyze clinical trial data, generate safety summaries, or support CMC submissions. Key requirements include:

  • Documentation of AI system development and validation methodology
  • Description of training data, including data quality and representativeness
  • Performance metrics across relevant subgroups
  • Ongoing monitoring plans for deployed AI systems
  • Human oversight mechanisms and override capabilities

This guidance does not replace existing GxP requirements. It layers on top of them.

Computer Software Assurance (CSA) Draft Guidance

The 2022 CSA draft guidance shifts FDA thinking from prescriptive validation protocols toward risk-based software assurance. For AI systems in GxP environments, this means:

  • Validation effort proportional to patient risk and software use
  • Documentation of intended use and risk assessment
  • Testing strategies matched to software complexity and risk
  • Continuous monitoring rather than point-in-time validation

CSA is a draft, but FDA investigators are already applying its thinking. Companies that continue building traditional validation packages for AI systems are over-investing in the wrong artifacts.

Predetermined Change Control Plan (PCCP) Guidance

The 2023 PCCP guidance, initially developed for AI/ML-based Software as a Medical Device, is increasingly referenced in pharma AI contexts. It establishes a framework for pre-approving anticipated algorithm modifications, which matters enormously for adaptive AI systems in manufacturing quality control or pharmacovigilance.

21 CFR Part 11 and Data Integrity

Every AI system that creates, modifies, or uses regulated records must comply with Part 11. Audit trails, access controls, and electronic signature requirements apply to AI-generated records the same way they apply to manually created records.

FDA 2018 Data Integrity guidance reinforces ALCOA+ principles: Attributable, Legible, Contemporaneous, Original, Accurate, Complete, Consistent, Enduring, Available. AI systems that auto-generate batch records, deviation reports, or pharmacovigilance narratives must meet these standards.


ISO 42001:2023 — What It Actually Provides for Pharma

ISO 42001 is a management system standard for AI, applying the same structural logic as ISO 9001 or ISO 27001 to artificial intelligence. It provides a framework for establishing, implementing, maintaining, and continually improving an AI management system (AIMS).

For pharma, ISO 42001 is valuable because it provides structure that FDA guidance does not. Where FDA guidance focuses on what AI systems must demonstrate, ISO 42001 addresses how organizations govern AI at the enterprise level.

What ISO 42001 Does Not Provide

ISO 42001 does not replace GxP requirements. It does not address 21 CFR Part 11 compliance. It does not satisfy FDA validation expectations on its own. A pharma company that achieves ISO 42001 certification without integrating FDA requirements has a management system that will not protect them during an inspection.


The Integration Framework: Where ISO 42001 Meets FDA Requirements

The practical question is: how do you build one governance program that satisfies both? The answer is to use ISO 42001 as the management system architecture and FDA requirements as the content that fills it.

Step 1: AI System Inventory and Risk Classification

Both ISO 42001 (Clause 6.1) and FDA guidance require you to know what AI systems you have and what risks they present. Build a single inventory that captures:

  • System name and owner
  • Use case and intended use
  • GxP classification (does it create, modify, or use regulated records?)
  • Patient safety risk level
  • FDA guidance applicability (which documents apply)
  • Current validation/assurance status
  • Monitoring approach

This inventory becomes the foundation for both your ISO 42001 AIMS and your FDA compliance program. Maintaining two separate lists is the root cause of most pharma AI governance failures.

Step 2: Risk-Stratified Governance Tiers

Not every AI system requires the same governance intensity. A classification model that supports manufacturing QC decisions needs more rigorous governance than an AI tool that drafts internal training materials.

Step 3: Align Documentation Architecture

ISO 42001 requires documented information (Clause 7.5). FDA requires validation documentation, batch records, and change control records. These are not separate documentation systems. They are the same documents organized to serve both purposes.

An AI System Governance Record (ASGR) for each Tier 1 or Tier 2 system should contain:

  • Intended use statement (ISO 42001 context + FDA intended use per CSA)

  • Validation/assurance documentation (CSA-aligned testing strategy)

Step 4: Governance Structure

The practical structure that works in mid-size pharma organizations:

  • AI Governance Committee: Cross-functional (Quality, Regulatory, IT, Clinical, Commercial). Reviews and approves Tier 1 AI systems. Meets monthly.
  • System Owner: Business owner accountable for each AI system. Signs off on risk assessments and change requests.
  • AI Quality Manager: Quality unit representative with AI-specific expertise. Reviews assurance documentation. Approves system releases.
  • IT/Data Science: Technical accountability for model development, validation, and monitoring.

Step 5: Change Control and Algorithm Management

AI systems change. Models drift. Training data is updated. This is where pharma AI governance most frequently fails.

Every change to a Tier 1 or Tier 2 AI system, including retraining on new data, threshold adjustments, or algorithm updates, must go through documented change control. For systems where algorithm modifications are anticipated, a Predetermined Change Control Plan should be established upfront.

The PCCP framework is the right model: document the types of changes anticipated, the performance boundaries within which changes can proceed without additional FDA interaction, and the monitoring criteria that would trigger a PCCP review.


GxP-Specific AI Governance Requirements

The intersection of ISO 42001 and GxP creates specific requirements that go beyond either framework alone.

21 CFR Part 11 for AI-Generated Records

If an AI system generates electronic records that are required by FDA regulations, Part 11 applies. This includes:

  • Batch records generated or modified by AI
  • Deviation reports where AI contributed to the analysis
  • Pharmacovigilance narratives generated by AI
  • Stability data analyzed by AI models

Part 11 compliance for AI systems requires attention to audit trails that capture not just user actions but AI decisions, access controls that distinguish between AI system accounts and human user accounts, and electronic signature workflows where AI-generated content requires human review and approval.

ALCOA+ for AI Data Integrity

ALCOA+ principles apply to AI systems with particular force on the Attributable and Original requirements. When an AI system generates an analysis or recommendation, the record must be attributable: it must be clear that the AI system, with version and configuration documented, generated the output. When training data is used to develop a model, the original data must be preserved and accessible for audit purposes.

Supplier and Third-Party AI Governance

Most pharma companies use AI systems from third-party vendors: laboratory information management systems with AI features, clinical data management platforms, pharmacovigilance software. ISO 42001 Clause 8.6 addresses supply chain considerations. FDA expects quality agreements and supplier qualification for systems that affect product quality or patient safety.

The practical requirement: your vendor qualification process must extend to AI features. A quality agreement with a LIMS vendor that does not address the AI-powered anomaly detection module is incomplete.


Inspection Readiness for AI Governance

FDA investigators are asking about AI during GMP inspections. The questions are still inconsistent across investigators, but the trend is toward more AI-specific scrutiny, not less.

AI System Inventory

Investigators ask: what AI systems do you use in GxP processes? Companies without a current, accurate inventory fail this question immediately. The inventory described in Step 1 above is your answer.

Validation Documentation

For any AI system in a GxP context, investigators expect to see documentation of how the system was validated or assured. The CSA framework is the current expectation, not IQ/OQ/PQ for software, but a documented risk-based assurance approach with testing evidence proportional to patient risk.

Change Control

Investigators look for evidence that AI system changes, including model updates, go through documented change control. An AI system that has been updated without a change control record is a significant finding.

Human Oversight

FDA is consistent on this point: AI systems in regulated contexts require human oversight. Investigators look for documented evidence that humans review, approve, or can override AI decisions. An AI system operating in a fully autonomous mode for patient-impacting decisions is a governance gap.

Training

Personnel who use or oversee AI systems in GxP contexts must be trained on the AI system and on the governance requirements. Training records must exist and be current.


Practical Implementation Roadmap

For a pharma company starting from a baseline of no formal AI governance program, here is a realistic implementation sequence:

Months 1-3: Foundation

  • Complete AI system inventory across all GxP and patient-impacting processes
  • Perform initial risk classification using the tiered framework
  • Establish AI Governance Committee with charter and meeting cadence
  • Define AI policy (top-level ISO 42001 Clause 5.2 policy statement)
  • Identify gaps in existing validation documentation for Tier 1 systems

Months 4-9: Core Program

  • Develop AI System Governance Record template and complete records for all Tier 1 systems
  • Implement change control procedures for AI systems
  • Establish monitoring programs for Tier 1 and Tier 2 systems
  • Update quality agreements with third-party AI vendors
  • Deliver AI governance training to affected personnel

Months 10-18: Maturity

  • Address gaps identified in internal audit
  • Pursue ISO 42001 certification if strategically appropriate
  • Develop PCCPs for AI systems with anticipated algorithm modifications

  • Conduct mock FDA inspection focused on AI governance

The Most Common Pharma AI Governance Mistakes

After working with multiple regulated organizations on AI governance, these are the mistakes I see most often.

Treating ISO 42001 as Sufficient for FDA Compliance

ISO 42001 certification does not equal FDA readiness. They address different things. A company that achieves ISO 42001 certification and considers its AI governance complete will have a serious gap when an FDA investigator asks about Part 11 compliance for AI-generated batch records.

Applying Traditional Software Validation to AI

Writing IQ/OQ/PQ protocols for machine learning models is a category error. ML models are stochastic, not deterministic. They require performance-based validation against statistically valid test datasets, not protocol-based testing of specific outputs. The CSA draft guidance points in the right direction. Companies that have not adapted their validation approach to AI are over-investing in the wrong artifacts.

Ignoring Model Drift

Validating an AI model at deployment and never reviewing performance again is not compliant with FDA expectations or ISO 42001 requirements. Models drift as the real-world data distribution diverges from the training distribution. A pharmacovigilance model trained on pre-pandemic adverse event patterns may perform differently as reporting patterns evolve. Ongoing monitoring is not optional.

Incomplete Change Control

Every AI model update is a change. Retraining on new data is a change. Adjusting a decision threshold is a change. Companies that have robust change control for traditional software frequently have gaps for AI system changes because they think of model updates as operational maintenance rather than controlled changes.

No Human Oversight Documentation

Having a human review AI outputs is not enough. You need documented evidence that the review occurred and what criteria were applied. An AI-generated pharmacovigilance narrative that was reviewed and approved needs a documented approval record, not just a verbal confirmation that someone looked at it.
